Pesticide Licence Application Forms

Application for a Pesticide Licence: To apply for a Pesticide Licence for retail sale of Domestic, Commercial or Restricted label pesticides, for the application of pesticides as a service / business, and for non-service pesticide uses prescribed by the Integrated Pest Management Act Regulation.

 

Pesticide Use Form to Obtain a Confirmation

Pesticide Use Notice: Form is used to submit pesticide use notice information to obtain a confirmation for pesticide uses prescribed by the Integrated Pest Management Act Regulation.

 

Pesticide Notices & Signs

Pesticide Use Treatment Notices: Model forms for use by pesticide user licensees to notify people about pesticide applications. These forms may be completed on your computer and then printed, or printed and then filled in by hand. If the completed forms are to be posted where they will be exposed to rain, they must be laminated or otherwise protected before they are posted.
  • Treatment Notice (PDF/215 KB) - meets the requirements of the Integrated Pest Management Regulation for posting in and around residences and outdoor public use areas.
  • Treatment Notice (PDF/406 KB) - meets the requirements of the Integrated Pest Management Regulation and WorkSafeBC (if workers can read English). For posting in and around buildings and outdoor public use areas.
  • Fumigation Notice (PDF/434 KB) - meets the requirements of the Integrated Pest Management Regulation and WorkSafeBc (if workers can read English). For posting when fumigant gases are used in buildings, containers or other enclosures.
  • Storage Sign (PDF/64 KB) - for use on pesticide storage facilities. After printing, laminate the sign to make it weather resistant.

 

Pesticide Sales Reporting Forms for Vendors

Pesticide Sales Record: For pesticide vendors to record information for each sale of Commercial and Restricted labelled pesticides.
Annual Summary of Reportable Pesticide Sales: For pesticide vendors to record total quantities of Commercial and Restricted labelled pesticides sold in a calendar year, to be submitted by April 1 in the next calendar year.

 

Pesticide Use Reporting Forms for Licence, Confirmation & Permit Holder

Pesticide Use Record: For licence, confirmation and permit holders to record information about each pesticide application.
Annual Report of Pesticide Use: For licence, confirmation and permit holders to record the total quantities of pesticides applied in a calendar year, to be submitted by January 31 in the next calendar year.
