Permit or Approval Application Forms

As the process covers large and small projects, the pre-application meeting is essential to setting the scope of the application process requirements. The Introduction also leads applicants through the guidance documents on Completing an Application Form, Consultation, and Technical Assessment.
The Environmental Management Act Waste Discharge Authorization application form consists of eight parts. The type of application being submitted determines the parts each applicant is required to fill out. A brief explanation is provided in each section below.
1. Permit Approval Application Form
This form must be completed by all applicants.
Application for an Authorization to Discharge Waste under the Environmental Management Act.
2. Environmental Protection Notices: New Application
This form must be completed by applicants who are applying for a new permit.
Generally, this form is not needed for applications for an approval.
3. Environmental Protection Notices: Amendment
This form must be completed by applicants who are applying for a major amendment to an existing permit. Generally, this form is not needed for applications for an amendment to an existing approval.
4. Site Plan
This form must be completed by all applicants.
5. Location Map
This form must be completed by all applicants.
6. Consultation Report and Referral Letter
All applicants for a permit must prepare a consultation report. This document will discuss recommended activities for the applicant to take prior to submitting an application for a permit, significant amendment or an approval. Here is a guide to completing this report.
During the pre-application meeting, ministry staff will identify the appropriate agencies with which to consult. Here is an example of an agency referral cover letter.
7. Technical Assessment Report
The Regional Manager will indicate to the applicant during the pre-application meeting whether or not a technical assessment will be required. Applicants who must complete a technical assessment should refer to this report. This document will discuss the recommended content of a technical assessment report for submission by the applicant as part of the application for a permit or a significant amendment.
8. Application Fee
Applicants must determine and submit the appropriate application fee.
